Although it's kinda obvious that youre not done with this form yet, I'm just gonna comment on everything I think you might need to know while filling the rest of this out...
For body type feel free to just put like skinny, fit, etc.
Make sure you include 3 flaws.
Life before CHB and RP example have to be at least 5 sentences.

By RP example, you just need to demonstrate how you would RP on this site. An example of RPing would be "I kicked the bucket." or "I walked up the hill." Basically it just you saying/describing what youre character is doing. If you go through some character forms on here you can get more examples/ideas for it
