I was wondering if I could leave my mortal parent as unkown because I planned for my OC to not know who they are because they died the day he was born and he's been moved from home to home.

Yes, that would be fine. However, it's preferred you at least put a name there and maybe a description -- since the form reflects what you know of your character (unless it's from their point of view), rather than what they know of themselves.
